Remote Monitoring
Up to 4 Self-Monitoring Devices based on veteran’s condition with “alerts” and notifications for “at risk” readings
Healthy Food
Condition-tailored nutritious Food Boxes delivered directly to doorsteps each month, plus up to $6,000 in healthy food incentives per year and SNAP enrollment support
Education
Education focused on managing and improving the relevant chronic conditions
Virtual Clinical Support
Monthly behavioral health coaching and medical nutrition therapy group classes
Suicide Prevention
24/7 access to suicide prevention resources and one-click direct connect with a live crisis hotline directly through the application (a real person trained to support veterans).
